Back-to-School Immunization Reminder

Back-to-school season is the perfect time to review your child's immunization record. Ohio law requires certain vaccinations for school attendance. Please check your child's records to ensure all required immunizations are up to date before the start of the school year.

Required Vaccines by Grade Level:

• Kindergarten: Students must be up to date on all required childhood immunizations.
• 7th Grade: Students must receive the Tetanus, diphtheria and pertussis (Tdap) and meningococcal (MCV4) vaccines.
• 12th Grade: Students must receive their second dose of the meningococcal (MCV4) vaccine.

Where to Get Vaccinated:

1. Your Health Care Provider
   Contact your child's health care provider to schedule an appointment for any needed vaccinations.
2. Morgan County Health Department
   Vaccinations are available by appointment only. Please call 740-962-4572 to schedule an appointment. Extended clinic hours will be offered on Thursday, August 13th. Please call to reserve an appointment.
3. Shriver's Pharmacy will hold a walk-in clinic for 7^th and 12^th grade students at the McConnelsville Pharmacy location on Thursday, August 13^th from 9:00 a.m. to 6:00 p.m.
4. Morgan Middle School on August 17th from 4:30 p.m. to 6:30 p.m. Shriver’s Pharmacy will be providing Tdap and meningococcal vaccines for 7th and 12th grade students.

A parent or guardian must be present in order for child to get their vaccines

Please bring your insurance card and your child's immunization record (shot record).
